Insurance company billed directly by the provider or should I file claim ? it's mentioned that 'Direct billing depends on the provider'. In the atlas insurance document (link below), it talks about the process to file claims. "You must submit a claim for any expenses to be paid by us. This includes treatment or services for which you expect the medical provider is to bill us directly. Assuming the patient goes to a provider (doctor/hospital) who usually bills the insurance company (or whatever third party it is), should I still file a claim? When are we expected to file a claim?
